Position Overview
We are seeking a compassionate and dependable Palliative CNA to join our growing team. In this role, you will provide personal care and supportive services to patients in the home under the supervision of a Registered Nurse or therapist. The ideal candidate is caring, reliable, and passionate about helping patients maintain comfort and dignity in their own homes.
Key Responsibilities
- Assist patients with personal care, including bathing, grooming, hygiene, dressing, mobility, and transfers.
- Follow written care instructions developed by the RN or therapist.
- Assist with exercises, ambulation, range-of-motion activities, and therapy-related support.
- Support medication self-administration as permitted by state regulations and agency policy.
- Observe and report changes in patient condition to the RN or Supervisor.
- Maintain accurate documentation of care and patient condition.
- Provide household services essential to patient care and comfort.
- Build positive, professional relationships with patients, families, and team members.
- Participate in patient care conferences and interdisciplinary team communication.
Qualifications
Required:
- High school diploma or GED.
- Successful completion of a state-approved nurse aide training and competency evaluation program.
- Active CNA certification in good standing.
- Valid driver’s license and proof of current automobile insurance.
- Reliable transportation.
- Ability to participate in on-call rotation.
- Not listed on the OIG Excluded Provider List.
Preferred:
- 1 year of CNA, home care, hospice, or palliative care experience.
